What should I do before my scheduled office visit?

For your convenience, we provide all required patient forms online. Please download the forms below, print them out, and bring them to your scheduled doctor's appointment.

Billing Policy

Please provide us with your complete insurance information and bring a copy of your insurance card with you to your appointment. As a courtesy we will submit a claim on your behalf.

Co-Payments

If your health insurance carrier requires that you make a specified co-payment for each office visit, it must be paid at the time of the visit. Co-pays will not be required for 90 days after a surgical procedure.

Referrals

Some health insurance providers require that you have a referral from your primary physician before being treated by a specialist. We must have the referral in our office before we can treat you. Please be sure you are familiar with our provider's policies before making an appointment with us.
